The End of Life Option Act became effective in California in June 2016. The law requires that you and your physicians take certain steps and complete certain forms to qualify for a prescription for an aid-in-dying-drug that you can self-administer when you are diagnosed with a terminal illness. This package includes a checklist and all the forms you will need to act in accordance with this law.
The End Of Life Declaration documents are supplemental to the Advance Health Care Directive and incorporate your wishes regarding end of life options to be implemented if and when you are terminally ill. If you do not qualify for the End of Life Option law and desire euthanasia, fill out the Euthanasia declaration form to state your wishes to be transported to a location where it is legal.
